Needle Sizes for Testosterone Injections

Testosterone esters are dissolved in oil[3]— thicker than water, which shapes every equipment choice. The common pattern is two needles per injection: a wide one (18–21G) to draw, and a fresh, finer one to inject — 22–25G for intramuscular, or a fine subcutaneous needle for subQ protocols. Your prescription's instructions come first; this page explains the reasoning behind the sizes.
Why does needle depth matter?
Intramuscular means the needle must pass through skin and the fat layer into muscle; subcutaneous means it should stop inthe fat layer. Needle length is what decides which layer you actually hit — and the fat layer's depth varies a lot by person and site, which is why length guidance is tied to body weight and injection site.[1]
Representative adult cross-section; subcutaneous depth varies widely by person and site. Skin thickness (~2–2.5 mm average) per Frid et al. 2016.[4]
What needle for intramuscular testosterone?
IM injections go at 90°, with length chosen so the tip reaches muscle through the site's fat layer. CDC/ACIP administration guidance for adults puts the deltoid at 1"–1.5" depending on body weight and sex, with the anterolateral thigh as the other preferred site; a 5/8" needle is considered adequate for IM only with the skin stretched flat.[1] For gauge, 22–25G is the standard clinical range[1] — with oil, the practical trade-off inside that range is simple: finer needle, slower (steadier) push.
| Job | Gauge | Length |
|---|---|---|
| Drawing from the vial | 18–21G | 1" (any — it never touches you) |
| IM — deltoid | 22–25G | 1"–1.5" by body weight (CDC) |
| IM — outer thigh / glute | 22–25G | 1"–1.5" (common practice; CDC tables cover the deltoid) |
| Subcutaneous | 26–29G | 1/2"–5/8", 45–90° |
Clinical ranges per CDC administration guidance;[1] the subcutaneous row reflects the fine-needle pattern validated by the FDA-approved subQ auto-injector[2] and common self-injection practice (community-reported, aggregated from reddit, MESO-Rx, and similar forums).
Can testosterone be injected subcutaneously with a small needle?
Yes — this is clinically established, not just community practice. XYOSTED, an FDA-approved testosterone enanthate auto-injector, delivers weekly testosterone subcutaneously through a fine needle.[2] Many prescribed TRT protocols use subQ injections into belly fat with short 26–29G needles. The trade-off with oil in a fine needle is push time: the same dose simply takes longer to inject, and very viscous preparations can be impractically slow through needles finer than 29G (i.e. 30–31G). Whether the depot behaves differently in muscle versus fat is a prescriber conversation, not an equipment choice.
Why switch needles after drawing?
Pulling thick oil through a 25G needle is slow, so drawing with 18–21G saves real time. Users also widely prefer not to inject with a tip that has already pierced the vial stopper, reporting it feels duller going in (community reports aggregated from reddit, MESO-Rx, and similar forums). Drawing wide and injecting with a fresh, fine needle gets you both: fast draw, comfortable injection. See the gauge chart for what these sizes actually look like.
